Order deCODE's AF™ Genetic Risk Test for Atrial Fibrillation

To sample and test a patient is a straight-forward procedure. Download the necessary forms, and ship to us with sample (blood or Bodetech buccal swap, provided by deCODE on request) as outlined below.

Please note that at this time we cannot accept orders for tests from residents of: California, Maryland, New York and Pennsylvania, Although deCODE's genetic laboratory is CLIA registered, state licensure enabling deCODE to process tests for residents of these 4 states is still pending.

Please note that tests can be directly ordered only by physicians and not individuals.



  Complete the forms
  • Have your patient read the Informed Consent form. Once they have had a chance to ask any questions about the test, understand it and wish to have it performed, the patient can sign the Informed Consent form.
  • Have a witness co-sign the Informed Consent form.
  • Complete and sign the Request form.
    Note: By signing the Request form you agree to the Test Submission Agreement (page 2 of Request form)

  Collect the sample

  Shipping
  • Check to see that the Requisition and Informed Consent forms are complete.
  • Check to see that the sample label matches the information on the Requisition form.
    Note: Incomplete documentation or incorrect labeling may invalidate a sample for testing or result in delays in testing and result reporting.
  • Place the blood tube in a larger screw-top plastic tube or padded Ziploc bag on a cold-pack, and then inside a properly padded box or mailing envelope. Buccal swabs can be shipped at room temperature and do not need a cold-pack.
  • Enclose the completed and signed Requisition Form in the box or envelope with the sample. The Requisition form should if possible be placed at the top, inside the padded box or mailing. envelopebetweensample/cold-pack liner and the outer packaging. .
  • Label shipping box/envelope on the outside as follows: “Do not Freeze - Store at 4°C upon receipt”.
  • Mail immediately by FedEx 2Day delivery service using the provided FedEx/deCODE account number on the Request form, to:

Sampling Guidelines


    Buccal sample:

  • Obtain Bodetech Buccal DNA Collector by contacting deCODE genetics Diagnostic Laboratory Client Services at diagnostics@decode.com or 1-877-222-6510.
  • Use gloves to protect the sample from contamination: Remove the Buccal DNA Collector from the pouch, holding the handle at base. Move slider cover back if necessary to fully expose the collection paper.
  • Instruct your patient in the use of the Buccal DNA Collector. It is important to observe that patients press the Collection Paper flat against cheek and that it is dragged accross at least seven time: Click to view/download detailed guidelines for the use of Bodtech Buccal DNA Collects
  • After sampling, push the slider cover forward towards the tip of the collector, covering the Collection Paper.

    Blood sample:

  • Label the sample tube with the client’s initials, date of birth (mm/dd/yy), sex (M/F) & date of collection (mm/dd/yy) in this order prior to sampling.
  • If the sample is submitted anonymously, label the tube with the unique Personal Identification Number (U-PID) that you have assigned it, as well as sex (M/F) and the date of collection (mm/dd/yy).
  • Use a 9 ml, standard lavender top EDTA vacutainer tube (either glass or plastic are acceptable), for sampling. Try to fill the tube completely. Note that a minimum of 5ml of blood is required for successful testing*
  • Refrigerate at 4°C immediately after sampling
